As providers of physiotherapy, sports massages and Pilates we are required to keep records of all patients we may treat. This includes medical information including medication and medical history. Along with this we have your general demo-graphic information (address etc)  

Permission from a parent or someone holding parental responsibility is required under the age of 16 years.  

Where adults with incapacity documents (AWI section 47) is in place this will need to be presented and copied before treatment commences. We are required to retain your medical and treatment records for a minimum of 7 years after the last full year of treatment, however timescales may vary where medical capacity is a factor or in the case of the treatment of a child.  

Your right to be forgotten cannot override the requirement for medical notes to be held for the minimum period as stated above.  

We do not use your information for any other purpose than that of what we provide as a service and where required to share with other care professionals such as your GP.  

Your demographic information may be used to provide you with appointment reminders. Your information will not be sold to any third party. You have the right to a copy of the information we hold about you.
